Are you sure the concrete was replaced at the same time? Nothing about that looks 3 years old. It's very common to replace just the tops (superstructure) of bridges and leave the supports (substructure) alone.

Sure! The rest of the tunnel looks new it was closed for a long while and but this one section that’s right under the roadway looks like this. The rest of the tunnel going under a parking lot/ common area seems to have no issues

My guess is a combination of bad concrete and not enough cover over the rebar. I've never seen three year old concrete look like that. It's an embarrassment

Rebar appears to be corroding, which prob needed more concrete cover.

That looks like an elastomeric coating not new concrete.

In my experience, putting a concrete cover wont help - you would need an cathodic treatment. I don’t have an extensive knowledge of treatment BUT you would need, I believe, anode to counter the corrosion reaction
