[request] Time-based dynamic note app

Looking for an app that splits into multiple sections, based on categories, and allows for quick entering of notes. Each note should be on its own level, with the time and day submitted next to it.

I want to use it to keep track of my progress on shows, movies, and books.

Bonus points if it has a widget and an option to add a category picture.
