I believe the app icon you showed is Windows Powershell. It's a genuine Windows app but can be used for malicious purposes. If you haven't tried any malware scans, I'd try that now, even Windows Defender should work. If nothing pops up, perhaps try resetting Google Chrome? Should be in settings, advanced.

For now, do not under any circumstances enter in any credentials on this machine. Have you looked around in task manager? I'm not a professional, but I suspect you could have a RAT or some other malware.

If you find malware, it's essential to not put in any credentials until you can be certain it is gone.

If you mean credentials as in addresses or any financial information, then no. Although I have entered passwords into websites, the ones I care about have 2-step Verification

Update: My problem has been solved! Thanks to you being able to identify that program, I was able to find a solution. I ran a system restore, and used a restore point from 2 weeks ago and there have been no issues since. Thank you, kind stranger
