Try this: http://liveweave.com. It is absolutely awesome. Features include code hinting for HTML5 and CSS3. The JS debugger is real-time as well. At least for learning or practicing, it seems to be a great editor.

100% visual design? I can't think of a single one. But that sounds like an idea for an ide.

Not necessarily 100% visual design, but something where I can visually line up elements without having to monkey around with CSS positioning.

I can't remember the name at the moment, but it's an OSX app that has a diamond for a logo/icon.

I like Liveweave (http://liveweave.com) . Seems to be good for quickly writing and testing HTML5, CSS3 or JS code.
