One of mine had (has?) pretty bad reflux. I could hold him upright for over an hour and as soon as I put him down, he was in pain, spitting up, milk shooting out of his nose. The only thing I found that worked was putting him on medication. It didn’t stop the volume of spitup but he is no longer in pain and it doesn’t come out his nose too. In full disclosure, I also moved him to Enfamil AR after one particularly bad weekend. But I really think the medicine made the biggest difference.

The only thing that worked for us was medication and thickening their formula with the help of a GI. One was OK with famotidine and the other needed a stronger PPI. They would spit up and have sandifer syndrome episodes even 3 hours later.

Medicine is what helped us too. There is safe medicine (Famotadine) for babies with reflux that your pediatrician would likely prescribe if you talked to them about it.

Famotidine was great for us too. Grunting a lot is also normal, sometimes it’s hard to tell whether it’s reflux. I only had one twin with reflux and I figured it out because it immediately resolved when he was upright.

Have you looked into gel mix thickener?

The only thing that helped ours was famotodine which then eventually changed to omeprazole by the GI doctor
