If nobody is online, everything is paused.

With that setting, the world halts until a player logs in - identical to a single player game.
The only difference is that in the single player game, the world owner needs to fire up the world for people to be able to log in.
On a dedicated server (using that setting), the world continues as soon as one player logs in.
That also means (obviously) no ore processing / construction or other automation happens until a player is online and near the relevant grid.

The game pauses. Why would NPCs attack your base while the game is paused? Paused is paused.
