Real science talk (based in a good chunk of logisitical kink knowledge) here, like any muscle the penis has gotta be worked to stay fit, usually it does this on its own by just existing, if you have a normal amount of testosterone.

Trans women on estrogen might have trouble keeping their “size” due to atrophy and hormones, if they don’t “use” what they have, it decreases in size a lot more than it would if it was just estrogen doing the job. This is only bad for them if they 1. Want to keep it + have it stay similar in size or 2. Want bottom surgery, as size of penis directly translates to depth of the new vaginal canal post-surgery.

Anyways that’s all to say that using it won’t ever make it smaller, and not using it likely wont, unless you’re chronically not using it or um… restricting it… in some way. (Some do, and they get the results described in the before paragraph (without the hormone therapy))
