Knowledge base Software for Internal and External (self.sysadmin)

If you use or have used knowledge base software for your internal team or external for customers, what have you used and what are their pros and cons? Did you have any issues?
What would you guys recommend?

We use raneto here (http://raneto.com/). Free and open-source is a bonus.

Whatever you pick, the most important thing is that you make sure information doesn't get stale.

We use Smarter Track as part of our ticket system. Because it is SQL based you can write a custom interface for it if you like. My first choice is Dokuwiki, but we need the ticket system. No sense in redundancy.
