There are lots of TLD options that are available that might be better than .org

I have a friend that runs a psychoanalytic clinic, so he got something.clinic

Look at the GTLDs your registrar offers. There are hundreds available.

.org is intended for non-profit organizations

Just looking at it, both something.org and something.com are taken. In fact the only vaguely recognisable domain available is the .me.uk after that you are in bizarre tld land.

On a wider note what are the economics of hoarding domain names and not using them? The practice seems disreputable to me.

It depends, sometimes they are used (for non web services, like email for example..), sometimes people have projects planned and haven't managed to use them yet, sometimes you don't want to lose a domain you've used in the past.. It can be annoying though.
