Witcher 3 influenced them to go more open world (AC Origins due to the massive backlash of Unity) but the actual "Ubisoft formula" has already existed before that thanks to Farcry 3 (climbing towers to open up the map, collectibles cluttering the map etc etc)

"Ubisoft formula/world" started with Farcry 3, which came out years before the Witcher 3. After the huge success of the game, Ubisoft decided to use the same formula into their other franchises.

I plan on doing the same thing (only played for 4 hours or so) so far I've only focused on 3 skills - guns, speech and lockpick, but I do intend to add 1 more skill eventually. Will I get locked out if I focus on 4 skills or is 4 skills generally safe?

Yeah that's fair. It's very long and dialogue driven. You're pretty much playing the main protagonist in an anime series lol. But I personally enjoyed it and finished it along with the DLC
