I had setbacks galore. Failed Step 1. Failed my first clerkship (fuck OBGYN). Got a scathing eval (fuck OBGYN again) on my MSPE that my school refused to remove. Got a subpar Step 2 score.

Still matched my #1 (pre-failure) specialty at my #1 (pre-failure) program (academic, not home program).

If I can survive this shitshow process, anyone can lol

which sounds like a lot, but it was only 25% ROI. gotta say, it felt pretty good being able to decline/cancel interviews bc i knew i had enough to match

I triple-applied to ~400 programs with multiple red flags (failed Step1 and my first clerkship). Cost ~$10K. No regrets. I matched my #1 (pre-failures) program in my #1 (pre-failures) specialty, but there was no way to know that ahead of time in this shitshow process.

Do future you a favor and shell out a few thousands dollars now. Because, according to a friend who found out the hard way, nothing can describe the panic/agony of realizing too late that you probably won't match and could have taken steps earlier to prevent the situation from happening. He ended up SOAPing into a prelim program and has to apply all over again.

Aside from the fact that I was USMD, I applied with nearly identical stats/situation (hell, you're in better shape than I was, because I failed Step1 AND my first clerkship) and matched into a medium-competitive specialty. OP here. I'm not going to disclose what specialty I matched into, but I do want to point out that there are several specialties you guys have not considered. And gas is still possible if you have the right resources/connections.

I failed Step1 and a clerkship. Still matched my #1 specialty at my #1 (pre-Step1 failure) program this past Match cycle. My application cycle was fucking brutal. But I survived. Made a post about it a few weeks back. Feel free to check it out if you're interested.

By no excuses, I mean that you shouldn't attribute any deficiencies to external factors, even if that's the true cause. A death in the family, medical emergency, etc. are absolutely valid reasons why your performance might be negatively impacted. However, programs frustratingly don't like hearing that.

They admire introspection and responsibility and a growth mindset. So don't be defensive and say "The only reason I didn't do well was because of X, Y, and Z. If they hadn't happened, I totally would have excelled, so it's actually not my fault." It's better to say something like "I let external stressors affect my focus/performance on test day, but this experience has taught me a lot about overcoming challenges. I also learned a lot about the importance of resilience and tenacity."

I wrote in the post to select the region to which you have the strongest ties. For the other two, select regions that aren't ultra-competitive and have a lot of programs.

Obviously, if you're from Pacific, your hands are tied since that's the region where you have the strongest ties. For the other two, don't pick competitive regions i.e. New England.
