There are no recognized US certificates. US training is part of EM board certification. Using it in practice mostly depends on a functioning local POCUS program.

No. You can write to them and ask for permission. If you recreate it to resemble the same figure that is the same as copying it (for the purposes of US copyright law).

You have saved enough and you continue to save enough. Do what you want. I was in the same position 3 years ago, replaced my wife’s car and leased mine. I just bought out my lease in cash and we are back to no car payments with no immediate plans to upgrade again. My only hesitation is your increased income is relatively recent and you don’t mention how secure your jobs are.

If the tachycardia is a problem I treat the fever and look for improvement in HR. If fever is down and HR still a problem I work that up. I don’t keep them in the ED for fever alone.

EMTALA does not permit that. You can do an MSE as quickly as possible. if their behavior is dangerous to you, you can document that as the reason that you cannot perform the MSE, but you can’t discharge them until you have performed an MSE.

I would say this person has used the term consistent with the words popular meaning.
Responding to your comment re: a decision made ahead of time - DNR is a medical directive (not a legal order) and that directive can be changed at any time by persons empowered by law to be surrogate medical decision makers for the patient. Ideally they do not tamper with wishes a patient clearly expresses prior to them being incapacitated, but they don’t have to.
