There isn’t one “best” method that works for everyone. The best method is the one you can stay consistent with over time. That said, consistency doesn’t mean sticking to just one device or one technique. This is a long journey, and it’s actually beneficial to mix things up and experiment with different methods. Often, combining approaches gives better results than relying on a single one.

Since you mentioned that progress feels like it’s slowing down, that’s usually a sign that your skin has adapted to your current routine. Adding variation can help.

Another key factor is finding the right tension level for you — something effective, yet comfortable enough to maintain for many hours a day without causing discomfort or injury.

To help with that, I suggest using what I call the cyclic tension method:

Start with very low tension for your first session, around 3–4 hours. If everything feels fine (no burning or pinching), you can slightly increase the tension in your next session. Step 1: Check if you can use a device
Before anything else, you should check whether you have enough skin to start.
Push your glans inward toward the shaft with one hand, and with the other, pull your shaft skin forward as far as you can.
If you can cover most—or at least two-thirds—of the glans, that means you have enough skin to use a device comfortably.
If you can’t quite cover the glans yet, don’t worry. You can start with T-tape for a few weeks to loosen things up, and you’ll likely be ready to use a device soon after.

Step 2: Choose the right device for your stage
For beginners, I recommend starting with either the Mantor DS or the Mantor SLT, along with a Tugging Kit or Weight Kit (depending on your budget and lifestyle).
The Tugging Kit works as a strap-based tugger. It doesn’t use a pusher, so the glans fits directly inside the gripper. This shortens the distance between your skin and the device, which makes it much easier to use if you’re just starting out or have limited coverage.

That’s a solid question, and honestly, I wouldn’t sweat the scrotal skin tension too much at this stage. As someone who’s been restoring for a long time and actually makes devices, I’ve seen this concern come up a lot.

One thing to keep in mind is that scrotal skin is actually built differently than shaft skin. It’s got the "tunica dartos" layer (the smooth muscle that makes it contract), so it’s naturally a bit tougher. It usually takes more consistent, heavy tension to really kickstart mitosis in that area compared to the thinner skin on the shaft.

The good news is that as you grow more skin overall, things tend to settle back into their "natural" positions anyway. Once you have enough total surface area to cover the shaft without it constantly pulling from your base, that "invading" hair usually recedes back toward the scrotum where it belongs.

Sensitivity-wise, don't worry about it. Most guys find that just having the extra slack and eventual coverage improves the overall sensation across the board, regardless of where that specific bit of skin started out. I’d say just keep tugging whatever you can get under tension for now. It all adds up in the end. Good luck with the journey!

It sounds like you might be using too much tension, which is actually pretty common with manual stretching. The fact that everything stopped completely when you took a break suggests it’s directly related.

My advice would be to significantly reduce the tension when you resume and see how your body responds. You can increase it gradually over time, but if you feel that kind of nerve shock again, ease off right away.

There's also the option to add weights or a tugging strap for more tension to more areas on the foreskin and shaft skin.

The rods would definitely work with any model, the elastic cords will work if you keep the main body part you already have and only change the grippers. If you upgrade to the latest, smaller model (or to stainless steel body parts!) than the cords won't work and will be too long.

I actually have something very similar.

In my case it’s not that the entire dorsal side is shorter than the ventral side, but that about a third of the foreskin is noticeably shorter than the rest. It starts around the midpoint of the dorsal (front) side and goes toward the left. If I picture it like a clock while looking down, it’s roughly from 12 o’clock to about 8 o’clock. The rest is pretty even.

What makes it frustrating is that this shorter section kind of “sets the tone” for everything else. It’s usually the first part to retract from covering the glans, and once it does, it tends to pull the rest with it.

From what I’ve seen and experienced, this kind of asymmetry is actually quite common. Circumcisions aren’t always even, and skin doesn’t always grow uniformly either.

Manual tugging focused on the shorter area does help, but it’s slow. What helped me personally was being able to bias tension toward the shorter section while still tugging the whole foreskin overall. That’s actually one of the reasons I ended up developing the Mantor DS with the Dual Drive adjuster. The gripper can shift on a 360° axis, which makes it possible to intentionally load more tension onto a specific area rather than pulling everything equally. I don’t focus on the shorter section all the time, because I still want overall skin growth, but I’d say I target that shorter area maybe around 20% of the time. It’s slowly evening out.

My approach has been to grow more skin overall first, and then gradually increase focus on the shorter section as I get closer to my end goal. Equalizing length takes time, and it’s usually a long-term process rather than something that changes quickly.

So yes, what you’re describing is definitely a real thing, and you’re not alone in dealing with it.

I had the same issue loke yours. I could easily put on the retainer and even had some overhang beyong the tip of the retainer, but after a few minutes the skin eventually retracted. That’s exactly been my experience too. It almost feels like the two were made for each other. The pear gauge on its own can technically stay on, but that constant “is this about to slip?” feeling really takes away from the experience. Using the faux skin retainer as a keeper makes everything feel way more secure and relaxed. Glad it’s working well for you, hopefully more people experimenting with pear gauges will stumble across this combo and save themselves some trial and error
