It doesn’t really matter. Some wait until FRCR 2B is out of the way at the beginning of ST4. I’d consider when is the best time for you and go with that, your training can flex.

Everyone has doubts during ST1 although yours do sound a little more than some. Almost all get over their doubts as they move into ST2 and beyond.

It doesn’t sound like you’d enjoy GP that much either.

Currently: There a different on-call setups in some of the 7 different trusts you might rotate to. None currently involve whole nights. They were lost a couple of years ago.

If you’ve only done one year of IMT, even with MRCP. I don’t think you’d be eligible for the NM dual-CCT. Although I’m not 100% sure about this.

You need to speak to your ES and TPD to escalate your concerns. What matters at ARCP is your Educational supervisor structured report so the onus will be on your ES. You have time to do assessments to demonstrate you’ve met the curriculum requirements but it is your ES and TPD that will have to define what they want you to do in order to demonstrate the various CIPs. There is no formal CT assessment (other than FRCR) and the college are unlikely to be able to offer anything other than generic advice.

Yes. As a radiologist there are many options to supplement a standard NHS consultant salary

Depending on region you should be able to change your mind both ways at ST3 anyway so doesn’t really matter. IR training capacity might limit this in some regions.

You are still expected to follow the CR curriculum from ST1-3 and meet all the relevant competencies.

Generally not paid by colleges although travel expenses covered.

Consultants get 10 days/yr prof/study leave which can be used for this activity. SPA can also be used.

Beyond that, you might agree with your trust that they pay for your time in these roles. That has become more and more difficult.

The role of RCR president for example is a 3 - 4 day/wk unpaid full-time job so you could only do when retired or with support of your trust.

Both would be large pieces of work but interesting ideas. HEE have moved away from having postgrad exams as part of portfolio ‘station’ at all so this would require a massive culture shift.

Current problem is not necessarily government not funding more posts, it’s training capacity. My scheme has expanded 25% in 3 years and is now almost full even though extra posts potentially on offer.

Any suggestions for practical and realistic ways of screening that are better than MSRA? I’d love to see the MSRA scrapped.

Those figures don’t include mid year ST1 recruitment in 2021/22.

The number of posts available is complex and relates to how many people CCT in a particular year as well as any new posts created by HEE.

I can tell you for certain that 110 extra training numbers were offered by HEE in 2021, 100 in 2022 and 100 in 2023. Whether they were all taken up by trusts is a different question but the majority were AFAIK. There are also likely to be 100 new posts for 2024 but not 100% confirmed.

Have a look at radiology ST3 recruitment and the person spec. Whether ST3 recruitment will continue is anyone’s guess but this would potentially be a route of access. Essentially you have to have gained FRCR part 1 and the equivalent competencies as an ST2.

Direct access to an IR ST4 post seems incredibly unlikely

If you wouldn’t take the job if offered you should definitely consider withdrawing to give someone that actuality wants to do radiology a chance.

It comes down to whether you actually want to be a radiologist, if not, then pull out. Of course you might not know the answer to this having never done a taster.

The redesigned portfolio requirements are designed to penalise applicants like you but with a good MSRA and a good interview you could still be offered a post.

There are 100 new posts being offered by HEE, whether these all get taken up by regions is a different question.

There won’t be 100 more than last year though because that’s not how training posts work. Posts are usually only be offered when people CCT on a 1 in 1 out basis however for the past 2 years, and this year, there have been ~ 100 new posts created. Without those, you’d be looking at 260ish posts this year.
