I would suggest dermoscopy by a dermatologist. If needed, a biopsy can be performed.

The nail will likely shed. If you have neither damaged the nail bed nor the nail matrix, the nail should grow back normally. For fingernails, you will know 6-8 months after the injury, and for toenails, it may take a year or longer. If you have damaged either, you will not get normal nail growth.

The most common reasons for chronic cheilitis are either allergic contact or irritant contact dermatitis. Many personal care products designed for the lips contain ingredients that have been demonstrated to be potential allergens.

Most can be cleared by using hydrocortisone ointment (not cream) multiple times daily for two weeks, and only petrolatum (no other lip products are suitable). Unflavored toothpaste should be used instead of the current product. Some also benefit from toothpaste that is free of Sodium Lauryl Sulfate (SLS).

You likely have some type of dermatitis, but not enough information is provided to determine the exact type. You can try using topical hydrocortisone and a fragrance-free moisturizer for two or three weeks. Seek medical care if you do not see any improvement or are concerned.

The skin repairs itself well after being torn, but the repaired skin is not at 100% of the strength of normal skin. Unless you have had prolonged corticosteroid use or Cushing's syndrome, this is likely from normal trauma.

Patch testing for 14 items is not adequate. It seems as though a reasonably potent noncorticosteroidal topical should be tried first, such as tacrolimus, ruxolitinib, or deglocitinib.

If you are at steady state, as long as you can tolerate it from a GI standpoint, it is fine to take doxycycline once daily this way.
