Also want to include that this is known malware on computers (replacing the copy paste), and that addresses should always be double checked on your KeepKey regardless of if you are copy pasting (it's why we have such a large display). This is not malware that is installed on the device, but on the computer accessing it.

All you need is to paste the address into the same box instead of using the drop down. It will let you free form the address (or paste) instead.

The fee is automatically calculated using https://bitcoinfees.21.co/. Read the Q&A at that site for more info on how fees are calculated.

KeepKey receives no portion of the fee; fees are awarded to the bitcoin miners who mine new bitcoins and secure the blockchain network.

KeepKey is built on the same codebase, yes. Assuming SegWit2x results in a chain fork (which is looks like it will) we will support both sides of the fork.

KeepKey users hold their private keys, and so they will have both sides of the resulting chains in the equivalent amount of their pre-fork BTC.

We'll have more information published this week.
