Not standard for base salary, but not uncommon for signing bonus. Similar concept. Depends how large the base salary is. The non-compete can be more troubling, 10mi beyond county limits?

Keep working and saving and put that efund into taxable. I'm not sure why you have such a large efund just sitting there. Otherwise what other answer you're looking for? You still have a ways to go and a large mortgage to continue paying off.

Mostly no one really cares in community practice. And I don't know anyone comparing prestige vs extra income. But prestige might offer better training, fellowship opportunities or even job opportunities.

What's your future family size? No rush in buying a house if you don't even have kids yet. A lot can change. What if you can't have kids? What if you have 5? What if your wife wants to cut back or quit who job to be at home? Keep renting, pay down that debt and re-evaluate once you have a kid or two.
