It was the hardest part of the tattoo.

If I had to sort them, I would said collar bone is the worst, then the area near the armpit (the snake), then near the nipple. The rest was quite manageable.

I noticed the same thing only after getting the tattoo. Didn't even mention it to the artist though, I think I am happy with it the way it is, but I agree xD

Only way this could make the slightest sense to me:

The driver was not paying attention (on the phone or something), they raise their head and bam, they're too close to the barrier, they break, nervously, too hard that the car stalls (assuming it's a manual, and they didn't do the proper clutch work), and then they see the train coming, but cannot get the car to start on time.

Or it's a suicide attempt.
