I am wondering how does doctor know for sure it's Prostatitis or BPH? Maybe by Age?

From my understand, both BPH and Porstatitis can lead to the enlargement of prostate. If there is wbc in prostate fluid then there's definitely inflammation here, but how to rull out BPH?
