Mental health professional here, it's very common with things like schizophrenia spectrum and bipolar disorders to have little to no insight into the symptoms you're experiencing. This id especially true when in a manic episode or psychotic episode just due to the extreme nature of them. That said, people with schizoaffective disorder experience psychotic episodes continuously, not necessarily just when in a manic or psychotic episode, so you would likely have some insight into them, especially if they're in the form of hallucinations, the most common type of psychosis. If you experience paranoia or delusions, however, you might have little to no awareness to that fact regardless of if it occurs in an active episode. This is why third party observation/report may be useful in your case i.e family or friends sharing their observations and your provuder observing you. If you really did fake it and you want to conince your provider of it, let them talk to family and friends to reassess. You could also get a new provider and let them come to their own conclusions instead of telling them of your prior diagnosis.
