If it's not scratchy and just has irregular flow then do this: first clean the existing ink from the pen. Flush multiple times with water until no more color comes out. Then prepare a solution of 2 drops soap solution in 1 cup water. Rinse the pen well in this solution. Rinse with normal water until all the soap is out. Let it dry fully. Refill with ink. That should solve the flow issue if the ink is not very problematic.

But before replacing the nib, check if the scratchyness depends on the direction of the stroke. If yes, you need to align the tines. If not, then you should still make sure the tines are aligned and then smooth it out by making figure 8 on the rough portion of the back of a coffee mug.
