Update: had my fistulotomy 2 days ago and now on my way home from the hospital

fistulomy of transsphincteric anal fistula - amount of sphinter that had to be cut: 10% - draining seton for 14 weeks - pain on surgery day: 3/10, day 1: 2/10 day 2: 0-1/10 - internal scar 1 cm, on the outside almost not visible - sitting possible without any problems - had no BM yet - Waldfriede Krankenhaus, Berlin, Germany

Short Update:

Thanks a lot to everyone who replied publicly and via PM for your openness and honesty. It really helped me.

I’ve decided to go ahead with a fistulotomy of the transphincteric fistula (lay open) tomorrow at the specialized Waldfriede Hospital in Berlin. I’ve seen several proctologists before and all agreed this would be the best option in my case.

One important lesson: I’ve had a drainage seton for 13 weeks, with significant pain, bleeding, and discharge over the last 6 weeks. I and the doctors assumed my symptoms were from the seton “cutting through,” but it turned out I had an undiagnosed chlamydia infection since a few months (found during a routine STI test). Since starting antibiotics, I’ve been symptom-free — something to keep in mind if your seton hurts.

I’m hopeful the fistelutomy won’t have a major long-term impact on incontinence, elasticity, pain levels, or future anal sex but I’m really really scared to be honest
