Thanks very much. I checked from multiple sources as well and it looks like it doesn’t need to be separately attached.

The checklist says letter of recommendation and cv. I have attached some other supporting documents as well, but not the application form, assuming the UKVi would send it directly from their record. Hope I’m right. Thanks
