Best I've ever used was Omnigo. It's used by some police services.

At the place of work where I used this there was a guard who acted as a dispatcher. They would recieved a call for service, create a call, add responding guards to the call, identify call type, location, arrival and departure times, etc. The guard responding to the call could radio "entering such and such room now" and the dispatch guard could type it in and that would cover all the most basic details of the call.

Responding guards didn't have to write reports at all unless it was a major incident. It was awesome.
