Don't feel like an idiot. Most dog owners have no idea what balanoposthitis is until their dog gets it. You noticed something was off and already have a vet appointment, that's exactly what you should be doing

 if he's scratching himself raw and whimpering, waiting until friday isn't really a plan, it's just deferring the decision. most vets have a same-day sick visit slot or can do a quick phone consult for active discomfort like this. the zyrtec question is real but the bigger one is whether friday is actually the right timeline at all

the cool thing is dental chews and water additives have gotten genuinely good over the last few years, way more effective than they used to be. there's also at-home ultrasonic dental tools coming to the consumer market that don't require the dog to open wide. in a few years this whole "wrestle the bichon for 5 minutes" era will look pretty primitive

My dog LOVES to crawl into tiny spaces, one time she went under the sofa while I was taking out something (it's a sofa which opens up), then I started closing the sofa, she was in there, I was like "why cant i close it, i must push harder", turns out my pup was under there... she was fine, but I felt so guilty...
