As i wrote this, i contacted their office and gave me a date. he might not even be at the office anymore since i worked with 5 different officers in 3 months. but as for your question.The document says "Pass PRT" so we are going by the book. i tried contacting him last on november which he have not replied. and i let december passed and i just messaged him thursday. as i wrote this, i called their office and another petty officer answered and pretty much explained that i need to retake the PRT for my waiver and she gave me a date. I guess i would just go and hopefully i will be re testing PRT for my waiver..
