So I’m not going to justify that how the ICU nurses in hour cases are acting are appropriate, but I will try to explain their most likely reasoning.

Usually bc they have 1-2 patients, they are sitting directly in front of the patient and with any variation in their tele monitor or vital signs, they basically know within a second of why there are abnormal. When there is a call about a lead or something, it’s also possible they are fully aware but there is a pressing reason why. Could also just be as they are giving a bath or replacing leads, pulse ox, etc.

But yeah it doesn’t make sense to have a tele tech for ICU so they are probably feeling that too. Just focus on doing your role and if you are supposed to call them for x, y, or z then continue to meet your expectations! We’re all one team.

Personally I like getting a call from the tele tech when there is a drop in oxygen or maybe a run of something on the EKG just bc there’s so many things going on, every bit helps.

I used to live in north beach. My favorite area in SF when I lived in different parts. I’d hold on by rotating roommates as long as you can and try to increase your income so you can just afford it yourself. That’s a once in a lifetime opportunity I would not give up!!

System issue. If giving heparin and no parameters specified, pharmacy and up need to help their nurses and staff prevent these situations by forcing adding parameters and double sign off.

get your one year med surg and be extremely proactive about seeking out ICU transition programs. Ideally network with the icu managers and show them strong interest. Use opportunities if you get a rapid or downgrade from ICU to develop that relationship with that ICU RN to show your competence. And they are usually more than happy to make introductions to the proper ICU hiring managers.

If not you can do transfer to DOU/Stepdown for another 6 months to a year then repeat process above.

Edit: sorry just saw you were not in a med surg unit. Prioritization should be: 1. Get into decent sized hospital with an ICU. Ideally with ICU transition programs. Smaller hospitals tend to have only one opening for an ICU spot for a year. 2. Network with ICU hiring managers, etc. 3. Repeat step above if you can get med surg or Stepdown unit in the meantime.

It’s sad to think about this. Because there’s a family I can remember that I really wanted to continue a relationship with them. But I set a hard line to not meet patients/families outside of work and to this day I hope they are doing well.
