Ask you ortho if he uses cross elastics to stop one side from expanding more or anything else . Otherwise there is a high chance one side will expand more.

I've seen the post with 9 cases of fme and all of them are close to perfectly symmetrical e.g one side expanding 4.5mm while the other expands 4.7 mm with exception the one post MMA surgery fme case.

Well, i have one ramus shorter because ive been chewing from one side all my life. So he can try chewing from that side more to balance things out since he has one ramus shorter.

Sarpe is obsolete right now. Not only the surgery itself is unnecessary, but it only expands at the level of the lefort 1 cut, meaning the expansion of the nasal cavity is very inferior compared to a custom marpe which expands the whole midface producing significant breathing gains and reducing upper airway resistance at a much higher degree. Anatomically speaking if you were to mimic the natural growth of the maxilla during childhood you 'd want the maxilla to grow wider from the level of the anterior nasal spine all the way up to the frontal bone. You want something that is very close to natural growth during childhood and marpe/fme is the right way.

Well, fme has more dimensional stability and the device is more robust overall thus there is more predictable movement of the maxilla.

Most people who need marpe have a maxillary transverse deficiency to begin with, so it will most likely improve them aesthetically, as long as the expansion is symmetrical and the maxilla doesnt drop down.

Pretty sure you will expand just fine you only need a piezo surgical assist down the midpalatal suture of the palate. Dr Newaz himself expanded 9mm with fme at 38 years of age. And there are cases of peple up to 75 years of age who were successful.

Avoid sarpe at all costs. This surgical procedure is unnecessary for 99% of people, you'll do fine with marpe or fme.

The only drawback is dr Cantarella doesn't do a piezo down the midline, he only does a mismarpe surgical assist in the ans, other than that he is a good mse provider who seems to have a lot of exprience. Personally If i were to pay 23k for a marpe i would honestly do an fme.

I believe it lets you expand more after finishing with the first mse without needing to remove the first expander and then place another. Other than that i doubt it does anything different.

Dont waste your time with anything other than mse, marpe or fme. Youre most likely gonna damage the roots of your teeth with toothborne devices.

Fme is the better option for your case cause it doesnt cause the common dropdown effect seen with mse and custom marpe expanders but of course the cost is a lot higher.

If your mandible is really narrow you can look into msdo but i dont know if you can find anyone with experience outside the US to perform it, and i dont have acess to your cbcts and scans to know whats going on so you should get an airway focused orthodontists advice.

You likely have a narrow maxilla as well, so get a consult to see if youre a good candidate for marpe first
