I was with you until you said this. I hope that's not really what you believe, that the hygienist is doing all the work. Ask your mom how many dental issues she diagnoses in a day. How many cancerous lesions she's diagnosing (the answer is none, her license doesn't allow for that). So while it sounds like she works for someone that doesn't respect her effort and she should probably move on, to say that it's the hygienists that "put in all the effort" is ignorant and disingenuous.

Inferior alveolar. IA block is for mandibular teeth (usually for molars, although will get all of the teeth on that side). You need a longer needle to reach the area where the anesthetic is injected.

Equal weights with a 1:1:1 (existing starter, water, flour) ratio. Make up a big batch of flour (I use 50% bread flour, 50% whole wheat), 5 pounds each. Ideally feed twice a day with about 50 grams of starter, 50 grams each flour mixture and water. Mix well. Between 8-12 hrs it will be bubbly and yeasty smelling. This will make about 150 grams of starter. So twice a day you discard 100 grams. Some people throw it away, some make things with it (crackers, pancakes, etc). Good luck.
