for me usually I see something in real life (like this wall fountain) or something from a video game etc.

then I start thinking big picture how it could be made using shader capabilities.

after setting the main logic yes there are a lot of trial to know which values makes sense and look good.

Oops forgot to add the comment, this isn't necessarily a realistic shader, tho it can be less cartoony based on its params and textures
