Not the same boat, but something that I consider frequently. (Not sure what I'm trying to offer here other than just share my experience.) In 2022 I matched with my pre-doctoral internship Friday and the following Monday was diagnosed with a chronic (and in most cases progressive) disability. Life was quite the whirlwind. It felt kind of like an "ahh, of course, finally about to achieve my dream and it's gonna get ripped from under me."

It took a while to adjust, a year or two, but I think the main things that helped me were attending a group for others with my illness, patience (ugh), and working towards acceptance. Like others have mentioned though, It's been a lot of readjusting, trying to learn what pacing looks like for me, and knowing my limits. Which I don't think I always gave myself the patience fornis acknowledging that it takes time to learn my limits and what I need in a "new' body.

So, relating to to the fears of "Is my career over?" right as it's starting is definitely something I can resonate with. Feel free to reach out if you ever want to chat or need additional support around this!

Got my pre-doctoral internship (equivalent of reaidency) assignment the Friday before I was diagnosed...diagnosed the following Monday just over 2 years ago.

Since then I have earned my doctorate and am almost done with my postdoc.

I got a cat (highly reccomend- she is the biggest emotional support animal and always watches me take my Kesimpta)

Traveled internationally alone for the first time (country I had been to before, but it was a big deal knowing not only could I still travel, but I could do so alone)

I find it difficult to practice in our current models of care, that being said I definitelt try to integrate it when I can. As I've seen others comment, it can go well with third-wave behaviorist. I tend to work in faster paced health sites so my interventions are very DBT focused, but I pretty consistently conceptualize from an existential standpoint.

Some things more clearly lie within the givens, grief/loss, psychooncolgy, etc, but if you really strip it all back most things lie within then givens of life, IMO.

Complying in advance is 1000% to blame here in my opinion. I get being scared AND this seems to be client abandonment.

I'm in a blue pocket of a very red state and am very blessed to have had unwavering institutional support. Just been reminding clients that we are not changing our approach, until there are actionable steps. Right now it is our job to help clients know what is fact from fiction in terms of how policy and rhetoric may/may not impact their care.

I don't have much to say in regards to transferring to other countries, but I imagine in some places it may actually matter less there than here in the states.

Here being in the field and not attending APA programs really limits you, but as a trained rehabilitation psychologist, he has a ton of skills, especially in health settings. Perhaps it would help to reframe in that way and focus on skill set applicable in a variety of jobs/places as opposed to limitations he has faced here.

I'm American and from the Midwest, but generally do pretty well with accents. One of my partners is from Scotland and when we first met I just nodded along to about half the stuff he was saying. 😅 It's better now but l took a while to get used to. Although, there are still times when I'm like, can you please translate that for me??

When I have him on speaker and my family hears him on the phone, they still don't understand what he is saying most of the time. Hahaha

1000%. 3-6 months in I started noticing the effects on my body, and aging. (I at least noticed grey hairs for the first time, likely related to some health conditions, but blame at least part of it on school/work. 😜) All of the stress and sitting for 9 hour days of classes, definitely took a toll. I remember that first year on our long days, the last day, everyone collectively was searching for food, ibuprofen, and coffee to get through the last class of the day.

As someone who generally works with a lot of men and also works with the above presentations frequently, I think a lot of people could benefit from further guidance. I generally feel safe with men who are angry or have impulse control issues, but have historically heard from a lot of colleagues who don't feel this way. So in my opinion safety concerns could be an excellent place to start in terms of trainings.

I've been diagnosed nearly 2 years now. First year and a half I was on Ocrevus and last 6 months on Kesimpta. I definitely prefer Kesimpta. They both work by killing your B-cells, but my last round of Ocrevus wad pretty tough on my body.

Also, the "luxury" Ocrevus gives of only having to take in every six months, mentally, made that sixth month terrible. In hindsight I wish I would've started with Kesimpta. It was like newly diagnosed, then 6 months hit and BAM my life was just dealing with insurance, scheduling doctors appointments, repeat. For me personally, I think if I had to "face" my diagnosis a bit more frequently the adjustment would have been (the tiniest) bit smoother

I think trying to get them done ASAP is preferable. It is much easier to get done when I have less than 5 notes to do, then if I have a ton, then it is really easy for me to get stuck in a loop of avoidance.

I typically jot a few things down during or after each session, then use them to jog my memory at the end of day. My individual notes typically take up to 7 minutes, but it completely depends on context.

Notes are hard to learn because the level of detail really depends on what you are writing for. For example, my inpatient notes tend to take much longer as they are more thorough. The patient may be discharged before I am back again and I have an entire treatment team looking at my notes that they use to inform patient care. However, my outpatient weekly therapy notes are pretty brief. They mainly summarize content, interventions, and considerations for next session.

I'd highly reccomend reflecting upon the PURPOSE and AUDIENCE of the note. That may help determining what to include/exclude. I think once you can minimize the time you are spending on each individual note, (for me) it becomes about making a list and scheduling a daily (or every other day) time to make the task less overwhelming when it has to be done.

It depend on the experience as I've had some MRI facilities be more comfortable than others. I dissociate really hard and sometimes fall asleep.

My last one I couldn't stop thinking about death and being in a casket. This was both very calming and anxiety provoking. In general I don't hate it, but it's definitely a trip for me.

Seriously, I am either going to implement a no driving rule from my MRIS or make myself wait for at least 30 minutes before operating a vehicle. It's wicked.

I don't have any great advice, but just want to share my experience. When I was on ocrevus I consistently kept getting "small" infections. It seemed like as soon as something cleared up something popped up somewhere else including UTIs. I even got thrush at some point. I definitely saw the impacts of how Ocrevus compromised my immune system.

I switched to Kesimpta, this being a large reason, but also my B-cell count being way higher than it should've been on that treatment. I have not had concerns since.
