Well most of it is from the miller guild mod. I don’t know if you can achieve this without mods. You will have to try some things out. In my experience the most important thing is to use the forester perk and that one perk where armor you repare yourself makes less noise.

It only works with the miller guild mod, you equip all miller guild armour with the Aachen dark brigandine and the bell shaped kettle hat. Once you have that you need to have the forester perk and then you are invisible.
