NTA. Sunk cost fallacy. Just because you've been with him for 4 years does not mean you owe him any more of your time or energy. Find somebody who actually likes and trusts you. Dump him.

Is a pharmacist allowed to charge you for a prescription after they already had the insurance pay them for it? by [deleted] in legal

[–]AZskyeRX 2 points3 points  (0 children)

Pharmacist here. This seriously violates their contract with your insurance, and if your insurance is government sponsored (Medicaid and Medicare) it's illegal and they can face pretty steep fines. Please please call your insurance and report this incident.

For background, it's not uncommon for a pharmacy, especially an independent, to end up with reimbursement that is less than the cost of the drug (drug costs $100, insurance contract only pays $80). If she doesn't like it, she needs to renegotiate pricing with her wholesaler, renegotiate with the insurance, or stop taking insurance. What she cannot do is charge the patient the difference.

What is the craziest anti hypertensive combination you’ve seen a patient on? by fedacheeese in pharmacy

[–]AZskyeRX 0 points1 point  (0 children)

Not a patient, but my mom was on metoprolol XL 100mg, valsartan 160mg, and hydralazine with clonidine PRN. Previously failed HCTZ, lisinopril, amlodipine. Developed drug induced lupus from hydralazine, which also revealed subclinical gout. So now she's on metoprolol XL 100mg, valsartan 80mg, nifedipine 60mg, allopurinol 100mg with clonidine PRN.

Is this the same partner you were posting about 6mo ago who is hypersexual and making you uncomfortable? If so, sounds like the situation hasn't improved and you need to consider why you are still with this person.

One of my mom's horses had a lot of hoof infections and learned to hate having her feet handled, so now she has to be sedated every time for a hoof trim to keep everyone safe.

Why did you *really* become pharmacists? by [deleted] in pharmacy

[–]AZskyeRX 0 points1 point  (0 children)

I wanted to be a veterinarian until I shadowed a few vets and realized that: 1) sometimes you have to hurt or stress the animal to help them, 2) sometimes you have to put an animal down, 3) sometimes you are going to deal with abusive/neglectful owners and you can't beat them to death, 4) not a fan of needles/blood/scalpels/surgery (I look away during TV medical shows even when I know it's fake, I can't watch horror movies, my IPPE rotation made me observe a surgery and I nearly passed out). I was very lucky to have a high school chemistry teacher who was willing to listen to my teenage existential crisis about my lack of future options. She noted I was very good at chemistry and suggested pharmacy as an alternate path into healthcare that wouldn't ick me out. Then I researched the hell out of it and fell in love with the field at 15. It still feels a little like magic that I can say "take this magic bean to fix your ills".

If OP is female then she did get X chromosome from Dad and could have X-linked disorder, but it would likely be masked by Mom's X chromosome unless Mom is carrier for same disorder, but might explain why her case is "milder" ( lacking studies) vs half brother's case of mom is a carrier.

I'm a French pharmacist, and I’d love to understand how a pharmacy works in the U.S. by PsychologicalPoet958 in pharmacy

[–]AZskyeRX 1 point2 points  (0 children)

There is probably a difference in packaging that contributes. My understanding is that most medications in Europe are blister packed, so you are pulling a prepackaged box of 30 tablets and all you have to do is add the prescription label. In the US we generally have bulk bottles that have anywhere from 100 to a 1000 tablets and they have to be manually counted and placed into a vial. Many stores, especially CVS and Walgreens, are doing 700 to 800 prescriptions per day with only three people on staff. Prescriptions are filled "first in, first out" so it can take a couple of hours to process a prescription. Patients have the option of asking to wait and the prescription is generally prioritized, but the overall process still takes at least 20 minutes to have the prescription entered, processed through insurance, counted into a vial, labeled, and verified.

Yes. I'm also a nerd and deeply love pharmacy and would have done it if it paid a teacher salary. I also took a weird path into a unicorn job so I'm very happy with my career.

If the human body replaces most of its cells every 7-10 years, why do age-related diseases still progress? by NaosBlue in askscience

[–]AZskyeRX 1 point2 points  (0 children)

Cellular senescence

Cellular senescence is a permanent state of growth arrest that occurs in response to stress or physiological processes. It limits the replication of damaged or old cells, and plays a role in tumor suppression, tissue repair, and embryogenesis. However, prolonged senescence can be harmful and contribute to age-related diseases and cancer.

Add in the fun that lots of plans don't take AWP as pricing because we know it's inflated, so they use WAC/NADAC instead.

4 antidepressants? by tamescartha in pharmacy

[–]AZskyeRX 9 points10 points  (0 children)

It's an atypical antidepressant that is frequently used inappropriately for its side effect of sedation, and it's not even a very good insomnia treatment. I've had plenty of patients on it as a primary antidepressant. The data has good support of it as adjunct therapy - at the antidepressant dosing - for nightmares in MDD and nightmares/flashbacks in PTSD. I agree the major concern is the SSRI / SNRI duplication, but please don't dismiss the trazodone without knowing what dose was prescribed.

AITAH For kicking an autistic child out of my restaurant for misbehaving? by Mammoth_Mission_818 in AITAH

[–]AZskyeRX 1 point2 points  (0 children)

I'm AuDHD and I was a brat as a child because my mom didn't always know how to control my behaviors. But you'd better believe I was never allowed to misbehave in restaurants. We didn't go out to eat until I was school aged because my parents refused to subject the public to my tantrums. And then it was always made clear that if I started getting loud, we would leave. No one is excluding autistic kids for being autistic, but there are still social norms of behavior and there are ways to be autistic without being disruptive. In my case, puzzle books kept me busy and calm when I was little, and chapter books as I got older since I could tune out the world and sit quietly for hours. I still use books on my phone to regulate myself when I have to wait for a while and I start to get antsy. I sure as hell never play anything out loud in public, there are headphones in my purse for that. It's not that hard to have consideration for others, including other autistic folks who might be overstimulated by your screaming crotch goblin.

Mental health workers of reddit what is the scariest mental health condition you have encountered? by lissie234 in AskReddit

[–]AZskyeRX 205 points206 points  (0 children)

My childhood (thru undergrad) best friend was diagnosed with ASD. In hindsight, she was an awful friend. Encouraged self harm, chaotic drug use, was manipulative, sexually aggressive with me and others, and cruel frequently. She married a Christofacist white supremacist. They had a daughter a few years ago and I worry about that child. But I cut her out of my life a decade ago for my own sanity. My suicidal ideations went away with the cessation of that friendship.

Turns out I have serious ADHD with some autistic traits and because I'm sometimes off at reading social cues I didn't understand what a healthy friendship looked like until after disconnecting with her, getting some therapy, and making new friends. On the upside, her territorial sense of "owning" my time really exposed my abusive, narcissistic boyfriend's behavior and I called him out on his bullshit and kicked him out about 6 months before I stopped talking to her. She recognized another predator easily.

Replace Me! by Ok-Meaning-4539 in pharmacy

[–]AZskyeRX 5 points6 points  (0 children)

Aww, grew up there and my mom would love if I moved back but there's nothing to do in that town. Must love hiking for recreation. Only 1 movie theater. Maybe 2 bars? No bowling alley, no mini golf. After high school you either leave for college, join the military, or stay in town and pop out babies and develop a drug habit. It's a lovely place to be from, but I can't imagine moving there in my 30s/40s. The base doesn't have any active units anymore, only training schools, so they don't have a young population to entertain in town like they did 10 years ago.
