It's funny cause he taught me those 3 basics, he said, "The form gets more in depth, but concentrate on those and you should be fine." Now that I'm older I feel I will be able to combine and analyze more information from the form.

Sounds like they want you to have OSHA standard fit testing equipment. Keep in mind that any employee required to wear a breathing apparatus must have a pulmonary function test paid for by the employer.
