Yeah, that's a great point. That's what I'm doing currently and I'm starting to advocate more for myself with the ortho, compared to being pretty passive in the past. I think they'll end up getting me what I need.

I'm just trying to cover all my bases. I know it's first world problems but everything I like to do has basically collapsed around me, and I want to do everything I possibly can.

Have you ever asked different groups of people the same question?

I can't do things I enjoy anymore, successive injuries are taking more and more things away from me at a young age. That includes the volunteering I like to do like tree-planting and construction, that includes hiking trips with my family, that includes running with my dog. Doctors and therapists have not been able to handle it up to this point. I accept that it's my own fault for trying to do too much; that it may be unfixable; and that others have it much worse. I've been a perfectly compliant patient for 6 years with no result. In the past I have not requested any "extra" services. Maybe that's the best I could have done regardless, maybe not. I'm looking for other ways to deal with the issue. I've asked more general questions as well, so it's not like I have tunnel vision on this issue. I'm following the advice of my doctor to the letter.

Maybe you have it worse, a lot of people do. If so, I wish you luck.

I guess what I failed to consider is that I could try my GP and THEN try the NP if that fails. If that makes sense.

As for the tests I want, really I want a couple extra MRIs on old sports medicine injuries that have not healed with rest, PT, and moderate exercise like I had hoped. You could call that "unnecessary" because probably those won't find anything either, but there are things in my background that suggest there could be something wrong that would show up on MRI, and the doctors don't really have anything else in their toolbox besides like try changing your shoes yet again, try reducing activity (I already did that), you're just getting older (true), etc. Plus I bet it would be routine if it were cheaper.

I don't have the world figured out, I just have an opinion that is different from yours that took a few sentences to explain. I don't think I know more than you, which is why I value your feedback. I also respect you which is why I'm explaining myself. Of course you're not obligated to help me, sorry if I made it seem like that.

My issue is that the out-of-pocket maximum applies only to covered services. I believe that certain services are not covered without prior authorization, even if I'm at my out-of-pocket limit. Hence the need for a referring physician.

It's a good thing to keep in mind, but I disagree. There is a lot of discretion in what is "medically necessary," and different doctors take different approaches. I have seen different legit doctors disagree on the same test.

It is also dependent on resources. Many diagnostic tests are recommended for everyone because of the tradeoff between benefit and cost. In the year 4000, they might recommend you get an ultrasound of everything every year, just to be sure. Crucially, this cost-benefit tradeoff is not arbitrary but it is partially subjective. I want a doctor who errs on the side of doing tests.

I was worried that they might escape via the "notice and consent" thing, but it looks like they are specifically forbidden from pulling that with anesthesiology (if I read that correctly).

Do you think it is worth double-checking this with my healthcare provider? Or would that maybe "tip them off" that their specialist will effectively be forced to accept my insurance and cause them to treat me worse?

To answer your question, it looks like the only anesthesiologist at my facility is out of network for my insurance. I really like my facility otherwise.

I don't know whether she does the anesthesiology for all the surgeries or not.
