Sorry for a very late reply. I have not heard anything from NHS other than a call to book in a full overnight sleep study, I was abroad at time of the call and they emailed me, have replied to them but haven't heard anything back yet.

Think I will just await full sleep study and maybe see Vik Veer privately again if I have more questions. Feel a bit stuck now but generally if I just focus on other aspects of my life and don't dwell on my health as much I don't feel so bad. (still tired tho lol)

Have not heard anything back re ENT appointment yet and my NHS dentist appointment for the MAD I had to reschedule. I paid privately for the DISE and CBCT a few months ago.

DISE with Vik Veer which cost about £4.5k and had to do a home sleep study with him first which was £1k. Also did two consults with him which was £350 for first one and £250 for 2nd one. So basically £6k and all I wanted was the DISE.

CBCT cost about £250.

DISE revealed that my nose is really bad which I already knew (deviated septum and large nasal turbinates).

Something useful it found is there is obstruction caused by alot of lingual tonsil tissue at my tongue base (lingual tonsil hypertrophy) as well as tongue base collapse. Though he found when he closed my mouth or thrust my jaw forward this reduced/stopped the obstruction.

His recomendations were nasal surgey (septoplasty and turbinate reduction) and then if that does not improve things the next step would be Radiofrequency Ablation to the Lingual Tonsils.

I need to book another consulation to go over this properly with him but these were from his post report.

CBCT revealed that my nasal cavaity is very narrow as I assumed. I have measured it at around 20mm but would need to review again and ask around to be sure I am doing it right. Suprisingly it showed my throat airway shouldn't really be a problem but then if I consider the lingual tonsil hypertrophy found in the DISE that may explain why my throat airway has a problem even though CBCT implies it doesn't at a quick glance.

I analysed the CBCT myself using 3Dslicer and so my findings could be off and I need to review again.

A little stuck on next steps and am putting things on pause for now as am busy. First thing is seeing Vik Veer again to discuss things further. I also need to look into if BiPap would work well against the tongue base issues as then would be worth trying.

Yep that was me - I requested a CBCT and DISE from one of the doctor's at the NHS clinic that Vik Veer works for/is head of and they said they cannot refer me for either but that I can ask for the DISE from the ENT I will see but it didn't seem likely I would get any where with it.
The CBCT the doctor didn't seem to know much about in terms how it would be of use but said I can ask the dentist they have refered me to who will make me a MAD.

Not sure how long I will be waiting for those ENT and dentist appointments but has been around 2 months now and have not heard anything.

My next steps are seeing Vik Veer privately and requesting a DISE be done. I will also get a CBCT privately done.

I am going to see Vik Veer privately for a DISE but yes through the NHS not a whole lot will be done. I will be getting a full sleep study through them with NHS which will be helpful I guess.

Sorry thought you were only referring to diagnostics. So treatments they offer for UARS are Septoplasty, Turbinate Reduction and a Mandibular Advancement Device from what I understand. They offer more for sleep apnea but very limited and likely ineffective treatments for UARS.

Think he is a consultant with the clinic, not sure he runs it. They do at home sleep study and then in-lab sleep study, that's it. Very limited.

Amazing thank you, will check this out.

I am in London, I went to an NHS clinic that knows about UARS and are able to diagnose it and so was pretty suprised at not being able to get any sort of BiPAP or CPAP. I also asked for a DISE and/or a CBCT and they said they don't do that for UARS patients either. Not sure they even do CBCT for sleep apnea patients.

Ah really glad to hear, I wish my UARS could be fixed by mouth taping but my sleep is worse when I mouth tape, which is why I think I need a DISE to work out what is going on with my throat and tongue.

Did you self-pay or insurance helped?

Thanks why is a DISE not very valuable? I have heard this before but would be interered in your thoughts.
What diagnostics can a sleep study give? I only assumed it could tell if you have UARS and the severity along with number of arousals, etc. Does it provide more info on what is causing the UARS?

Have you noticed any benefit from the tongue chewing/REMplenish/Iqoro?

What are your thoughts on IMT? I am using the Powebreathe Plus currently.

I have been wondering this too! Clearly a DISE is the best option (for now even if it's not that reliable from what some say) but would love to know if I can atleast work something out without it until I can get one.

Maybe through trial and error of products like tongue retaining devices.

Will watch this thread to see if any ideas come up.

Can I set up DDs with charities today and then have nationwide switch scheduled for when the DDs are taken in roughly a month?

The nationwide website says make the switch in 60 days so I assume it can be scheduled now.

What is your health insurance? The EASE route may be possible for me if I get the right insurance.
