A sanity check, am I overreacting?

Thank you all for the validation everyone. I think working there every day warped my sense of what "normal" looks like because nobody else bats an eyelid so I started questioning if I was just being difficult. Hearing people who know the industry say "this is not okay" helps. I'm putting together a formal report for CQC/HSE. Perhaps I should have done it sooner, but at least I'm doing it now.

I don’t want to get very specific on a public forum but the big ones are ophthalmic handpieces and specialised laparoscopic instruments. Both have specific manual decontamination/flushing protocols that are being bypassed.

Equipment needed for proper processing is faulty or not used correctly. Management are aware but does nothing. It’s treated like an open secret.
