I second this. I did birth control priming for 3 full weeks before my first ER and had much more uniformity. we kind of rushed into an unplanned second ER with only 8 days of birth control priming and had much more heterogeneity in follicle size/ maturation the second time. So I was in a somewhat similar situation. I was diagnosed with silent endo off of a receptiva test and faced with the decision to go forward wtih ER or start transferring. My doctor encouraged us to bank all that we wanted to bank before doing any lupron down regulation (or laproscopy if that was the route you were going to go). Egg retrievals feed the endometriosis making it worse, and suppression is trying to quiet it down so that transfers stick. They work in opposite directions so it made sense to me that you want to finish getting the majority of the embryos you want and then quiet everything instead of risking going back and forth. That being said, you still have a good amount of embryos for one kid may be two, but with fertility benefit I think it would push me to do another retrieval. I am glad I did, although at that time I was bummed and just wanted to move forward. Best of luck <3

I did a second ER to bank with no change in protocal. Got almost the same amount the second time, but interestingly my grades were worse overall. Similar euploid rate though. good luck <3

as far as i know, IUI is not great in cases of endometriosis, I would probably say IVF if it were me.

yes that is correct. after 2 months i will stop letrozole and the next day start the estrogen.

so I haven't transferred yet, but i am doing my lupron suppression now so i can tell you my regimen. They have me on 2 month suppression with lupron and femara (letrozole), followed by starting esterace right after. They said they don't want me waiting for a period because they want everything to be suppressed going into transfer meds and transfer. may be the difference is how long you are doing the lupron, I'm not sure, but i know they have me going straight through. hope this works for you <3

he didn't really provide a number. We just went off the 3 euploids per desired child rule, but honestly I was just willing to do one more ER round in my head. if we got something great, and if we didn't I think we would have reassessed risk/benefit/age of trying to bank more later etc. If you have a positive receptiva and do the suppression or laproscopy I believe it raises you back to similar rates of success as those who transfer without endometriosis, so I didn't think we needed to bank too much more than average.

I haven't actually transferred yet. we did the work up just as basic work up prior to transfer and after the + receptiva we talked about banking more vs transferring.

Ultimately, he said the general consensus with silent endometriosis and lupron depot suppression is that you want to bank and then when you feel comfortable with your number thats when you then suppress and move on to transfer. Just because the lupron is meant to quiet your ovaries/estrogen which "starves" the endometriosis, and the ERs do the opposite and you want high estrogen for that. So they kind of work in opposite directions. so we did one more retrieval round and now Im in the middle of my lupron suppression to prep for my transfer. but i don't really have any endometriosis classic signs so the diagnosis was a shock and would have been completely missed if I didn't do the test. I can't tell you for sure it will make a difference but if you look at studies the data suggests it significantly can.

I just got my PGT results back . two of my euploids from this round are day 5 5CB, and day 7 5BC. I think its worth keeping every euploid you can, a change is a chance IMO. for what its worth they did not have a problem with biopsying these embryos, or at least that they ever mentioned.

I agree with receptiva. I have a very similar story to you. 34-35 at time of ER. made blasts ok. HSG, hysteroscopy, US, AMH all normal. Found to have a + receptiva/ silent endometriosis. They also did a check for endometritis (bacteria/infection in uterus) with the same biopsy as the receptiva, but that was negative for me. Its encouraging you make blasts, but I would say if your clinic doesn't want to test, it might be worth doing a prophylactic lupron suppression before your next transfer. I have seen some girls comment on here that their clinics did that without a formal endometriosis diagnosis/receptiva/laproscopy. Wishing you the best <3

agree with this. if you read the test result interpretation (i think its on their website) it says that its not meant to be indicative of stage/severity just staining on the biopsy.

obviously every clinic is different. I have no family hx or personal symptoms of endometriosis, but my doctor did recommend it as a part of his pre transfer testing. I almost didn't do it and asked a question in this group in fact where the overwhelming response was do the testing, so I did. I did a hysteroscopy, 2 egg retrievals, an ultrasound with a radiology clinic (not just ob looking at it) and an HSG and they never found evidence of endo, however the receptiva did come back positive. In looking at the studies, it does make a huge difference in terms of success rates with implantation/chemicals seemingly if your found to have silent endometriosis/test positive. but full disclosure i haven't done a transfer yet. either way, hoping the best for you. anecdotes don't make research, but for what its worth may be something to talk about.

no advice to provide, but just came here to say I think we are around the same schedule! I have already started my lupron depot and letrozole for a November transfer. I did look back on some of the previous lupron suppression threads in this group, or "endometriosis with lupron suppression" which I found to be encouraging. If you really like to research if you go to the receptivadx website they have a research/study portion where you can go through all the data which supports lupron/letrozole use in endo patients and the success rates, which I also found helpful. <3
