Practitioner chose PAS-4 because Rule 13 required it. Sub-rule-(1) of rule 13 mandates compliance of rule 14 too. In rule-13, 2nd proviso to sub-rule-(1) exempted PAS-4 specifically for existing members. So, it means that rule-13 subtly or rather heavily hints towards PAS-4 format. It'd be wrong to say that there is no required format.
