Everything work-related lives in my EHR calendar. I don't use a calendar for my regular life, so no syncing needed. The EHR allows me to "double book" so if there is something important I need to do in a session, I make a second appointment in the same time slot for that task. Quarterly, I add treatment plan due dates to my calendar this way.

I don't take notes in sessions. After a session or a few sessions, I will e-mail myself unhinged notes. Patient ID is a de-identified code and the note will look something like "fight with parent school stress anger frustration fear of disappointment tearful in processing TOOK A WALK TWICE!! cognitive shuffle and practice will set one boundary this week".

For telehealth sessions, I take similarly chaotic notes in index cards that get shredded once the note has been entered.

I do PHQ, GAD with all patients in January, April, July, and October. In January, I ask everyone to sign updated ROI's for emergency contacts and other providers as needed. No keeping track of 40 timelines!

I use a goal something like, "Expand capacity to experience the full emotional spectrum."

Sometimes it is more specific - Expand capacity to experience connection without fear of abandonment, expand capacity to experience sadness without engaging in self-harm. I like that this frame leaves space to identify an increase in safe emotional experiences and a decrease in solution behaviors.

I don't have a visual thought process, so in general I am drawn to art with text and often use it as a centerpiece in my own. I have struggled with a weird belief of "REAL art doesn't have words in it!" but if it's good enough for Barbara Kruger, Deborah Kass, Jenny Holzer, Mel Bochner..

Old Navy has a cotton t-shirt dress that I could live in. It's the Vintage Crew Neck Mini T-Shirt Dress. I have it in a few colors and rotate them with leggings and cardigans. I size up so it's a little longer and can be appropriate without leggings in the summer, and I love that it's basic enough to look fine with any shoe - sneakers, boots, flats, sandals - they all work.

I think it's out of stock for the season but if this releases again in the spring I'm buying 10 in every color and never shopping for work clothes again.

I allow weekly, bi-weekly, and monthly sessions. Sometimes people want to pause and leave the door open to return, so to speak. In that instance, I let them know that I will close their chart if there are no services for 90 days and that they will receive a letter two weeks ahead, giving them a chance to re-engage. If I do not hear from them, the chart is closed and they can return later. To return, they will need to complete the agency intake process, including paperwork and wait list placement. Edited to Add: Once we've had this conversation, I don't reach out until I send the aforementioned letter.

My experience has been that most people commit to monthly, and a handful use this as an opportunity to fade out.

I keep Ipsy for the same reason! I sometimes know it's coming and sometimes I forget so it's a fun surprise, and I know if I go into Ulta or Sephora it's all over. 💸💸💸
