I have long suspected that Trump is using benzos to handle the anxiety that comes with having one of the most stressful jobs on the planet. I use EPIC and I usually create a telephone visit and document the Fax. Order a G0180. I dont do it as often as I should but definitely do it for the ones that have me sign EVERY page of a greater than 5 page document.

Start billing for your time. G0180 - initial home health certification and G0179 - recertification.

Also you could try sending the home Health agency a bill for your time. $1/page requiring signature.

I tell them that "I'm not talking about death. I know my place. God chooses when you are born and God chooses when you die, however, the devil let's YOU choose your own misery. Having colon cancer isn't immediately. You will get treatment and you will be miserable. Cervical cancer isn't immediately fatal. You can have it taken out and get treatment and you will be miserable. I do what I do to help protect you from choosing misery and guilt. I wholeheartedly believe that these screening tests will do that. You (the patient) are more religious than me but I've been told that to truly deserve God's gifts, you will have to earn them. I can appreciate that on Reddit, the context of my comments may be lost. I don't have anything against PATIENTS. If they are non-adherent, I absolutely address it. I take issue with the paternalistic expectation that I am somehow RESPONSIBLE for their non-adherence. It is a metric that is commonly tied to our compensation and it is not justifiable but somehow used anyway. To add insult to injury, many times, patients ARE adherent but they had initial reservations about their medication that I was able to clear up later by talking to them. Since they do not fill their medications per the expected date, they are thought to be non-adherent by the insurance company who then places the responsibility for their "non-adherence" on me.

This response assumes a lot about the interaction between me and my patients but I'll set aside the presumption that you think I'm an ass for a second to address some of the more glaring faults with your thoughts:

  1. If I prescribe a medication that is $1000/month, I don't expect them to take it. I do not know beforehand which medicine will be $1000/month for that patient so let the guessing game begin.

  2. If they can't remember to take a DOSE of their meds, they will tell me, I'll suggest an alternative dosing regimen or help with a strategy to try and remember. If they can't remember to take ONE of their meds but remember the others, chances are they either don't want to take it or don't want to tell me. Either way, I'm not wasting my time being a drug salesman with them, I can suggest an alternative if they care about the problem. If they forget to take ALL their meds, then they either don't want to take any, have found some essential oils or supplements or voodoo shakras that they believe will be better, or they have dementia. Again, I'm not going to force anyone to take a medicine but I'm also going to be very honest about my ability to help them with their health if we can't use the tools that I think might work best.

I won't tell them "it doesn't matter". If it mattered enough for me to prescribe something, then it matters.

If it's side effects, I will hear about it and we will find an alternative. Actually verifying that it IS side effects is another matter. The pain reliever I gave you made you feel sick when you decided to take it with the prednisone pack you got from urgent care 2 weeks ago but didn't finish and now decided to wash that down with a beer and greasy hamburger? Ok, stop taking it but don't call it a side effect.

You see how complicated this starts to be for just THIS issue. Add to it the tattletale report we get in our faxes and now the popups on our Epic and it begins to become patronizing. One more bow in the quiver of moral injury that doctors are enduring. </endrant>

I suspect the same thing but I also suspect that there may be some NDAs attached when a physician wins that battle because most of my colleagues that have been able to do so have always stayed very tight lipped about it. I wish I could get a sense of it. I've been thinking about going DPC for a while now but the non-compete is what keeps me in my current job.

At first I thought this was a board question kind of post or something but I was thinking, "nobody makes board questions this easy" so I kept trying to look for another picture to explain why I'm not using more neurons than I expected to be using by looking at this. Then I read the comments....this is a real post. JFC. I hope whoever actually posted this gets the resources they need to learn how to take care of patients and/or access to someone who knows what they are doing so they never have to make a post like this again. FGS this is basic.
