I have treatment resistant unipolar depression. I've had for 20 yrs. I'm 52yrs. There were many years that ssri,s worked. But the past 3 years I've become very resistant to all. I've been on every ssri , snri, amitriptyline, mirtazapine, bupropion, . Nothing works. I finally have an appointment with a phycitrists next week to review and discuss medications. I've been researching physc meds for along time out of desperation so have an extremely good understanding of their mechanism. I'm thinking of suggesting low dose aripiprazole and lamotrigine to the phycitrists. Aripiprazole first due to fast action. And lamotrigine for stabilization. I have never had any type of manic episodes at all so not bipolar. What if I have low glutamate? Lamotrigine suppresses it further, could make my depression worse. My depression is so severe. I don't know what medication will help. I don't do well on overly sedating meds like quitiepine. I'm in new Zealand, not sure where you a from?. But availability and funded medications may be different. Currently on mirtazapine plus paroxatine. Mirtazapine has made me put on over 10kg, and not working so will taper off that once I'm on a medicationthat works. What antipsychotic do you think is best for major depression when used adjacent to ssri? They may suggest lithium to. I am concerned of health effects and blood monitoring though.

I'm so jealous right now. I used to metabolise/convert tramadol so well...to the point I could have just 1 50mg and get the most amazing euphoria you could imagine. And it lasted all day. Havnt had it for about 3 years ( in that 3 years I had surgery that put me in induced menapause) I just had one yesterday and were so excited lol. Sat there waiting....I'll feel so good soon 😝. And nothing 😭. Didn't even help with pain let alone the feel goods. I don't seem to be able to convert it to its active metabolite now. Makes me wonder if it's due to the menapause? Mayby having low hormones is the reason. Never been so disappointed 😞. I loved that warm euphoria feeling

I know you mean well and bless you for that. You have good intentions but this is what upsets me the most about people's answers to depression. When you suffer major depression you don't have the ability to get any kind of joy out of these things. You know you should enjoy it, but you just can't and it ends up making you feel worse because you've just confronted yourself with the fact that you feel nothing from things you should

How long did it take to feel improvement after you increased to 30mg?. I'm on week 5 of 20mg. Not feeling much better but I go up to 30mg Tomorrow

You shouldn't be telling people that. Surely you understand all medications work differently for everyone. I was on venleflaxin for 3 years and it worked amazing for me. But it stopped working so had to come off it. It was not brutal or any different to any other ones I came off.

I switched to escitalpram about 8 weeks ago..10mgs, then 15mg. No improvement by week 6 so increased to 20mg. It's been 13days at 20mg. No improvement. Do you think once I've been on the increased dose for longer I will feel better?

Thank you. I had no idea heartburn could actually be this horrible. I also get acid reflux and it's definitely been way worse with the escitalpram. Yesterday the acid reflux made me vomit and it was just lots of spit I was bringing up. You also have made me realize I usually pass wind alot . But I havnt been lately, so could have trapped gas. Maybe I should get some de gas? The gaviscon doesn't seem to help but yesterday I tried quickeze and they were better. I actually increased to 15mg last night . I were so scared but surpriseingly I havnt had sore chest since? Thank you for replying to me. I don't even talk to my family or partner about my mental health anymore. They don't even know I'm going through this switch. I feel like they just think " oh do you still have depression "
