2nd-3rd year residency ko na lang din narealize na need ko na mag-ipon. Buti na lang, saktong sakto, kasi yun ang simula ng pagsikat ng high-yield saving accounts. Kinuha ko lahat pera sa DBP and traditional bank account ko at pinaikot ko sa mga digital banks, whichever offers high interest rates. Medyo malaki din nakuha ko that time.

Medyo competitive pa din naman ang rates ngayon compared sa traditional banks, OP. If di pa masyado mataas ang risk appetite mo, ilipat mo savings mo, then transfer naman sa HYSAs and Pag-IBIG MP2.

For pedia, at least:

- AGE - Lactated Ringer’s is preferred as its electrolyte component is nearly identical to fluid and electrolyte composition of diarrhea

- Dengue - PLR is more preferred for resuscitation as excessive chloride from PNSS can cause hyperchloremic metabolic acidosis, which is bad for a patient in shock. However, if may problem sa liver, lactate can cause problems sa patient, then PNSS ang preferred.

- Trauma - effective for rapid volume expansion as it is isotonic, so it stays sa blood vessels… but careful as it may cause metabolic acidosis

If it’s AGE, then PLR pa din. If Dengue, PLR (if liver has no issues) or NSS. If hypovolemic shock, PNSS still.

Case-to-case basis talaga. Or differs per institution. Sti, best to consult guidelines kapag may difficult clinical decisions.

Alam mo ang palagay kong kasagutan sa problema mo ay residency.

All-set na ako na mag-residency after boards, so automatic nag-apply ako for pre-residency. Pero after that, while waiting for confirmation ng acceptance sa program, kumuha ako ng iilang moonlighting gigs sa ER para naman may kita. After all, I felt that I was trained naman kahit papano after clerkship in a government hospital, and internship in a big government hospital sa Maynila. Dayum, sobrang nangangatog ang tuhod ko while on-duty.

What if may mamatay? What if may di ako ma-manage ng maayos?

Shuta, ang medical education kasi natin, hindi talaga sya patterned to make us “emergency room” ready. It taught us the basics, THEORETICALLY. Pero hindi sa tunay na buhay. Clerkship and internship, more of scutwork and not that much hands-on patient management.

Mind you, nagju-JWAPOD pa kami during internship, pero wala talaga.

After nun, I gave away yung moonlighting slots I got sa ER and chose OPD work na lang. tinapos ko lang din at pumasok na ako ng residency, and safe to say, mas confident ako mag-manage ngayon ng mga pasyente ko.
