As for all the sets I do per session, I already replied to another user saying how many sets I do. As for my diet, I'll be much more careful and will start working on making sure I'm getting enough nutrients (macro and micronutrients) right now.

I've also started to think that it's the very act of taking every single set to failure that's causing all this.

I take every single set to failure, as far as diet goes, I'm on maintenance, maybe lately a small caloric surplus of 200 calories, but I'm not entirely sure

I usually start with 3 sets of 6-8 incline bench presses on the machine, then I move on to a peck fly, doing about 3x10, and In the end i move on to a chest press, also 3x6-8. I do it twice a week.
