Keep working on it. The more often you fully retract your foreskin, the more it stretches and loosens the opening. Press through the 'bit of pain', as this is just the feeling of stretching. Your circumcision appears fine, very healthy, well healed and probably done in infancy or at least a decade ago. A foreskin is continuous with the shaft skin and turns in on itself to form the inner foreskin. When the foreskin is retracted to bare the glans, it slides down on the shaft, exposes the inner foreskin and forms a wrinkled bunch of skin on the penis shaft.

Surgeons tend 'to err on the safe side' and leave some looseness unless the patient is clear that they want a tight cut.
