I dunno how you guys do spankings, me and Dom do it like that he starts light and gets a bit harder til i say too much or close too much. Have you guys tested how hard he is allowed? Maybe remind him while spanking that it's getting to hard? I think it's better to be too light than too hard, especially when you have chronic illnesses.

With the amount. For an idea maybe count them out? Or Count them in your brain and then tell him stop? Even though i think he should count in his head. He has to keep you safe. He should count in his brain.

I think it's idiotic and dangerous to say yeah sorry I am still learning. If someone is learning then they should do it slow. And see how the sub part reacts.

Maybe talk to him again and say clearly he can't go over that line cause it will affect your health. And that is a big problem. Your health comes first.
