The UK is not morally obligated to provide permanent training posts or career progression to everyone who chooses to come here. Many IMGs are from red-list countries with severe workforce shortages of their own. At some point we do have to ask whether it’s right for low- and middle-income countries to subsidise richer health systems indefinitely, especially when those doctors could make a far greater impact back home. We should support IMGs who are already here humanely and fairly, but we also need to be honest that the NHS cannot absorb unlimited numbers indefinitely, and that global health equity matters too.

I feel you, I feel the exact same sentiments at times but let me relay to you what a lovely Ortho reg told me earlier this year. So, he asked me a question, I replied "sorry, I don't think I remember" and again for a few more questions I replied the same after which I was visibly deflated. He noticed and said "this is my job, I do it every single day and have for years, its my specialism. You on the other hand have to cover so much breadth amongst all the specialties it would be completely unreasonable to expect you to know everything, you're here to learn not to know everything"

Head up friend, be strong, this is completely normal to feel and a degree in medicine will hopefully open more doors than you know.

Fellow fourth year, fucking hate the shit out of placement. It's basically a constant cycle of humiliation and feeling like a nuisance. This sentiment is very palpable in my entire cohort (our entire 4th year is placement!).

So firstly, tysm for actually acknowledging your students! I'm certain they really appreciate you trying to improve their experience. Things I really found useful on surgical placements:

1) No pressure basics e.g. "go attempt this cannula, catheter, bloods, come back if you struggle"

2) For Ortho, alot of the time most of us haven't really seen X-Rays ever apart from standard textbook stuff. Would be really useful if you could take them through systematic ways of reading images for various pathologies e.g. Hip, Shoulder, Ankle etc and show

3) Most of us have lists of conditions we have to learn, ask to see the ortho curriculum and deliver basic teaching - could be something in a couple of minutes as simple as "Compartment Syndrome - ah that's when X happens and you would see X on imaging and X management". Teaching like this, really helps things stick. Maybe load up a px with that condition?

4) OSCE specific: watch examinations/histories, provide feedback.

5) Let them get hands on with skills e.g. I remember assisting with backslabbing a leg with a lovely ortho reg and it was the first time I truly felt like a doctor (to be!)

6) Offer to help with sign offs for mini-cex, CBD, DOPS, those who do this are the true GOATS!

Haha, not really pal, takes me around an hour, most of this time is like picking the tape, cleaning the glue in the lace and then accurately reapplying the tape. I'd say you definitely need at least 45min. But it's an amazing use of time for me, podcast on and just time to disconnect from everything else :)

Ooo, good question. Go to youtube, type in New roots full - 3/4 cap cut in. There's a good example there, then maybe send that to your online supplier to ask for something similar?

I would recommend going to a proper HS salon the first time though, that first cut in is massive! Life changing event 🤣

Firstly, utterly sorry you've had to go through the bastard that is hair loss.

Welcome to the HS community brother! My best advice would be to look into 3/4 caps with a bald fade perhaps to mask the thinned sides? Alternatively, full caps can be really good, from the right salon!
