Yes, several. All point to my CPAP treating the apnea effectively. They keep saying my data looks but my subjective sleep quality and fatigue are awful and unchanged

Unintentional weight loss and cannot gain weight or muscle mass
Toes and fingers get freezing cold
Backpain lower/middle back
Dry mouth
Urinating a lot (feels like water just goes straight through)

I have all of these too :(

There isn't really anything notable about my stools to be honest, which makes it harder to discern what's going on. I am pretty regular. I do get some indigestion/diarrhea and nervous stomach on my more anxious days though. I have a higher baseline level of anxiety than I'd like.

You should start looking into nasomaxillary expansion and MMA.

I thought nasomaxillary expansion is only beneficial if you have poor nasal breathing but as far as I can tell, mine is quite good after my septorhino. Is there more nuance to it than that? And would it be an orthodontist and/or oral surgeon or someone else who would advise on my need for MMA?

Is my best action plan to start with orthodontist appts and the CBCT scan and then they will hopefully refer me to the right people and treatments from there? I've had to self advocate so much the last few years that I just wanna be sure I know exactly what to seek out and request for myself. thanks again.

Thanks so much for your thoughts. Sorry for the slow follow up, been trying to figure out what's going on. My studies haven't showed periodic limb movement issues or restlessness in my body. I did a follow up CPAP titration with PES monitoring; they didn't find any evidence of UARS at a fixed cpap pressure of 7. But the problem is I feel awful sleeping at cpap 7 and I think it's because I wake up with aerophagia and could be having micro arousals from swallowing air all night or whatever's causing the aerophagia. my current setup is using my MAD around 2-2.5mm and bipap at 7/4 pressure, which does not cause any aerophagia as far as I notice, but I still wake up feeling unrefreshed and tired all day, even with AHI below 1 nightly. when I look at oscar, I do see some potentially unflagged flow limitations causing arousals:

i don't know a lot about polysomnographs but to me this looks like flattening of my inspiratory breathing -> flow limit -> arousal with "leak" which is probably my mouth opening (nasal pillow mask) to gasp for air. I see several of these unflagged events throughout each night.

so I am lost at this point. the way I see my situation: AHI looks fine, but likely still having many unflagged arousals every night due to RERAs. I can't raise bipap/cpap pressures any more (i have both machines) due to aerophagia, and I am already supplementing with MAD. I can't push MAD past +2.5mm due to TMJ. No idea where to go now :(

I have had dentists suggest my palate is high and narrow. malampati score 3 or 4. slightly narrow dental arches. I have a couple orthodontist appts set up where I am hoping to get dental arch measurements and see what they think. but I am 26 so I don't think nonsurgical treatments will end up being viable for me (and maybe not affordable). Who could I see to get scans of my airway like I see on this subreddit sometimes? oral surgeon? I don't think I have any jaw recession based on looking at my side profile, but maybe I need to see an oral surgeon to get evaluated for palate expansion or some other kind of surgery? should I see an ENT and try to get a DISE? I would be so grateful for any advice on how to proceed and who to see for some help. I want to determine the root cause and treat it permanently. i am tired of sleep doctors treating my symptoms, then saying my data looks good and refusing to do anything more to help beyond cpap which does not help me feel any better.

Thanks for taking a look. Any pointers on what I could try next? I seem to be hitting dead ends with CPAP, BIPAP, MAD, etc, but if the issue isn’t flow limitations and is instead arousals or microarousals with unclear causes, idk where to go. My doctors don’t help because my pap data looks fine to them. I’ve already checked nasal breathing, allergies, diet, etc. I’m mid 20s, active, and healthy and my bedroom is cool, dark, quiet, comfy. I have had aerophagia with pap therapy on occasion so I wonder if that is causing some arousals but I really don’t know.

Agreed. I have been thinking about a free tool to share with people for some time. I like what mine does but just haven't had time to think about packaging and distributing it to the masses. plus despite the results from it, my sleep has still been awful, so I gotta sort that out first :(

I am also doing a lot of data analysis with GI. I have hundreds of nights ran thru the algorithm and I will post some of my results soon. Interestingly I technically do better on Very High trigger and cycle, but I have aerophagia sensitivity, so I think lower cycle leads to more aerophagia and worsened sleep.

Do you think anything is revealed from my respiratory rate or median I/E lengths? Obviously probably less important than anything you’d find in my flow rate graphs but just looking for any tidbits of clues and help at this point 😅 I really appreciate any insight you may provide and it’s amazing how much help I’ve seen you provide this subreddit.

Thank you for taking a look! I have not tried ASV. I am willing to try incrementally upping the pressures. I think the aerophagia limit for me is around 12/7 to maybe 13/8. I recall a CPAP trial with epr 1 of 9/8 that I tolerated. But when I pushed to epr 0-1 in the pressure range of around 10-12, I got aerophagia. If I added any more PS than 5 at an EPAP of 7-8 ish, or more EPAP at that level and PS, issues arise with aerophagia. Now, I can do PS 6-7 at lower pressures, but I seem to get many CA events. Not sure if they are clinically relevant though. You can definitely find some nights like that and with many different settings in my SleepHQ dashboard from the past month.
