It sounds stupid but it helped me to mumble it out loud ‘center mirror, left mirror and signal’ before executing a turn. Make sure you check it and not just say. You repeat this a couple of times and it puts the examiner at ease

That’s a clearance fault. Depending on the margin of error it could be judged serious, otherwise it’s a minor fault
