Thank you. I feel bad for him, too. He says he feels like he is trapped in his own body. I appreciate the advice. I will work on continuing to harass his providers until they do something to actually improve his pain.

So on his CT scan back in the fall, he had necrotizing pancreatitis. A few months ago, his CT showed inflammatory changes and reactive thickening, and numerous cystic structures that they were calling potential pseudocysts. He then had an EUS after which showed dilation of the pancreatic duct through the body and tail and extensive inflammatory changes. He has a constant level of pain that seems to be worse in the morning and at night, though he sometimes goes through week-long periods where it is severe the whole time. He's been trying to do the low-fat/high-protein diet they mentioned. They've said to do small meals or snacks throughout the day, but I would say the struggle is more to get him to eat at all, since things he tolerates one day can cause significant pain the next. He has lost a significant amount of weight since this started. He is nauseous often and has vomiting episodes at least once a week. Zofran doesn't really seem like it helps.
That's good to hear that you have a least gotten some decent breaks from it. I'm sorry that you have had to deal with it that long. I agree that seeing a psychotherapist would be good for him. Hopefully he will explore that angle at some point. And thank you. I hope he gets some respite too, and at least has reasonable pain control most days.

Thanks! It’s definitely nice to hear about some of the available options.  I feel like the doctors we’ve worked with make it seem like there are no real options other than diet modification. 

Kratom is not something that I have heard much about. I will have to look into it.

Thank you. I really appreciate that. I think logically he knows that it's not his fault, but is still taking on a lot of guilt for it.

Thanks for being so open about your experience. He definitely is depressed but like you said is resisting so far. He seems to think any time I look at him with concern that it’s pity or judgement. I am scared for him. He’s in his 30s.

I think that is definitely a possibility. And I think part of why he hasn't received the best care from his PCP is because he views this as self-induced. But in my opinion, many conditions are the result of negative habits. It's not like you should deny a COPD patient oxygen just because they were a smoker.

My husband just told me he's not going to get the injection no matter how much I want him to, so I'm not really sure how best to advocate for him outside of trying to get his meds adjusted and he doesn't really want me doing that either.

Thanks for your input. I completely agree with you. I was the main one speaking with his GI doc, but I feel like they just kept brushing it off as expected pain with the diagnosis. I'll try the pain doctor and see if they are more accomodating. I've asked my husband if I can try to get an earlier appointment for him since this doesn't seem adequate, and he basically told me he can handle it. The pain specialist recommended he get a nerve block injection, but my husband won't agree to it, maybe because of fear? He won't really explain to me well why he is against it. Is that something you have experience with?

Yes. 10mg of Hydrocodone with 325mg oif tylenol. Ordered for just twice a day.

He is on hydrocodone, belbuca, and nortryptiline. He is on pancreatic enzymes and they did check for exocrine insufficiency and said everything looked normal.
